How can I receive services?

Simply contact our main office at 443-759-9592 to schedule an appointment. Depending upon circumstances, we have been able to schedule an appointment on the same day or within a 72 hour period.

What is the cost of services?

Transformation Health accepts all brands of Maryland Medicaid (Maryland Medical Assistance Programs) and has an out-of-pocket sliding scale fee for eligible persons based on the Maryland State Poverty Guidelines and ability to pay. To find out if you are eligible to pay on the sliding fee scale, go to the "Billing" tab and complete the Financial Sliding Scale Agreement for consideration. Someone from our administrative department will contact you to discuss any possible arrangements.
