MINOR PSYCHIATRIC REHABILITATION PROGRAM
PSYCHIATRIC REHABILITATION PROGRAM FOR MINORS (PRP)
Transformation Health Psychiatric Rehabilitative Program for Minors is a licensed community-based mental health center providing quality rehabilitative and support services to minors with emotional disturbances and behavioral concerns. We service children and transitional age youth from ages 6 to 17yrs.

Designed to encourage self-determination of goals and improve minor’s ability to function at home, at school, and within the community.
Transformation offers weekly, bi-weekly, and monthly rehabilitative groups for our youth. Youth will experience an array of community outings.
Transformation Health has a full range of onsite and offsite services. To include assisting youth in developing life skills, conflict resolution, social skills, money management, promote activities that support the client’s cultural interest. Community awareness, time management, personal hygiene and grooming, mobility skills, age-appropriate boundaries, nutrition, trauma, and strengthen family interaction.
Initial assessments are completed by licensed clinicians, which determine their treatment level. Services are then carried out by our skilled and dedicated Direct Care Coordinators. Transformation Health has its own Mental Health Therapists or can give a list of providers in your area.
PROSPECTIVE CLIENTS MUST MEET THE FOLLOWING CRITERIA:

Have a mental health diagnosis and be inactive in mental health therapy

Be referred by a licensed mental health therapist

Enrolled or eligible for Maryland Medical Assistance Program
